Medalogix Nurture Allows Encompass Home Health to Identify Discharged Patients In Need of Additional Care

At the end of January, healthcare technology company Medalogix announced the release of a new case study with Encompass Home Health and Hospice Inc., "Leveraging Analytics and Clinical Transformation to Achieve 'A Better Way to Care.'"

Officials said the study demonstrates the dramatic results the Texas agency experienced after deploying all of Medalogix's analytics-based population health solutions throughout its organization. The study highlights Encompass's use of Medalogix Nurture, an analytics solution that helps identify and manage recently discharged patients who are at elevated risk of hospitalization and could benefit from additional care.

After only nine months of using Medalogix Nurture to streamline a discharged patient calling, or after care program, Encompass was able to facilitate more than 5,000 patient contacts, resulting in identification of 276 patients in need of additional home care.

"Nurture has helped us become more efficient by narrowing our focus on high risk patients who may benefit from touchpoints after discharge," said Encompass Vice President of Clinical Transformation Janice Riggins, RN. "Patient care does not stop once discharge has occurred. By quickly identifying patients through the Nurture risk stratification, we are able to intervene at different milestones to mitigate avoidable emergent care situations and provide the right care at the right time." 


In addition to Nurture, Encompass also began employing the full suite of Medalogix's predictive solutions, which includes Touch, which identifies patients at highest risk for readmission and automates a home health clinical team's touchpoints via Interactive Voice Response (IVR) calls, and Bridge, which helps identify and inform patients who would benefit from hospice care.

After deploying Touch, Encompass:

  • Decreased the risk of avoidable inpatient readmissions by more than 50 percent for the Complete Joint Replacement (CJR) and Bundled Payment programs. 

  • Decreased re-hospitalizations in Value Based Purchasing states by 32 percent.
  • Saved over $1M by leveraging automated calls instead of manual calls. 


After deploying Bridge, Encompass increased total hospice days by 74 percent over the course of 22 months.
