Capstone Pediatrics Earns Big Bonus

Capstone Pediatrics, a Medicaid Accountable Care Organization, recently earned a $397,000 bonus from UnitedHealthcare Community Plan of Tennessee for its successful care-coordination efforts improving the quality of care for people enrolled in UnitedHealthcare’s Medicaid plan. As a result of their work, Capstone physicians have reduced hospitalizations among UnitedHealthcare Medicaid patients by 28 percent. 

Capstone Pediatrics’ efforts to more effectively coordinate care for its patients had impact across the “Triple Aim” goals of healthcare – improving the patients’ experience of care, better health outcomes for the population, and lower costs. Capstone Pediatrics enhanced its capabilities by improving convenient access for their patients through increasing same-day appointments, integrating physical and behavioral health, conducting outreach efforts to encourage preventive health services, and establishing a team of care providers dedicated to providing exceptional patient care.

Capstone Pediatrics’ 10 pediatric and adolescent clinic locations throughout Middle Tennessee serve approximately 10,000 of the state’s UnitedHealthcare Community Plan participants, including many who have complex medical, behavioral and social-support needs.
